If your SIM (Surat Izin Mengemudi — the Indonesian driving licence) is lost, or the physical card is damaged so that it can no longer be read, you may apply for a replacement card at your local Satpas (the police driving licence issuing office) without retaking the tests, as long as the validity period of that SIM has not yet ended.

Procedure for Replacing a Lost SIM

  1. Report to the nearest Police office (Polsek/Polres) to obtain a loss report certificate.
  2. Bring the required documents to the Satpas.
  3. Have the SIM number data verified against the police database system.
  4. Complete biometric identification and the printing of the physical replacement SIM.

Required Documents

  • Surat Tanda Penerimaan Laporan Kehilangan (STPLK — the police loss report receipt) from the Police (if lost)
  • The physical damaged SIM (if damaged)
  • Original KTP (Kartu Tanda Penduduk — the Indonesian national identity card) and a photocopy
  • Certificate of physical and psychological fitness

What if the SIM is lost and its validity period has also expired?

If the validity period has expired, the procedure for issuing a new SIM applies and the tests must be taken.
